Lincoln Park CERT is a volunteer based organization that provides both emergency and non-emergency support and assistance to the Lincoln Park Police and Fire Department. Lincoln Park CERT is sponsored by the Lincoln Park Fire Department. Lincoln Park CERT provides assistance to nonprofit organizations sponsoring special events. Lincoln Park CERT DOES NOT replace professional personnel (i.e. Police, Fire, EMS, Security, etc.). Rather, Lincoln Park CERT provides supplemental assistance at special events. Typical event support offered by Lincoln Park CERT consists of basic first aid, eyes and ears, limited crowd support, event support and assistance along with incident command experience. The Lincoln Park CERT Board of Directors, Coordinator and/or city sponsor reserve the right to reject any request for service.
Form Instructions: Nonprofit organizations seeking the assistance of Lincoln Park CERT must complete the form below. This form is only valid for the dates listed and new dates must be submitted on a new form. The entire form must be completed. If there are additional documents (flier, map, etc.) which will assist in making a determination, please email those documents to info@lpcert.org. This form must be submitted at least 30 days prior to the first date listed on the form. Organizations who are approved for assistance, will be contacted to make further arrangements.
